How do I get a user id and password for Montlake's web site?

Log on to www.montlakeholdings.com

Click on the CLIENT RESOURCES menu item

Select GET ACCESS

Follow the instructions and a user id and password will be e-mailed to you shortly.

Montlake will assign a user name and password to each valid policy holder that requests one. The user identification and password are not specific to an individual user, but are assigned to a policy holder's organization. It is the responsibility of each policy holder to keep the assigned user id and password secure and to regularly change their passwords to ensure the data remains confidential. Montlake cannot be held responsible for the misuse of these assigned user names and passwords.

How do I change my password?

To change your password you must first log in to the web site using your old password. At the first menu screen select "Change your password" from the list of options. You will be prompted to enter your User ID, current password and your new password. You will be prompted to enter your new password twice to confirm it. Select the "Change It!" button. Once your changes are validated you will see a message confirming the change. Select "Continue" to return to the main menu.

Who is allowed access to the data on Montlake's web site?

All Montlake policy holders are allowed access to their claims data. Brokers and Agents are allowed access if the policy holder elects to provide them with their user id and password. Montlake will not provide this information to anyone but the policy holder.

How secure is my data?

All data is contained in a password protected Microsoft® SQL Server© version 7.0 database. The data cannot be accessed unless the user has a valid user id and password. For any data transmitted over the Internet, Montlake uses Secure Socket Layer (SSL) encryption and a digital certificate of authority provided by VeriSign®. SSL technology is the industry standard in security protocol. Using this technology provides reasonable assurance that your data is protected and secure.

Once I have a User ID and Password, how do I log on?

Navigate to www.montlakeholdings.com and click on the ON LINE menu item then select LOG ON. You will be prompted to enter your user name and password. Once your user id and password is validated, you will see the main menu of Montlake's on-line services.

What if I don't remember my password?

If you have forgotten your password or your password is not working, first check with any other users in your company to see if they have changed it. If you still need assistance, please call Montlake at 800-292-3496 and ask for web site support.

How do I view my loss data?

Once you have logged on to Montlake's On-Line Services you will see the On-Line Services Menu. You have several options. Those options specific to viewing loss data are described below.

  • List All Claims - This link will display a list of all the claims for all policies. Clicking on a claim number will open the detailed information on an individual claim.
  • Query Your Claims - This link will allow the user to filter the claims listed based on a variety of criteria. For example you can create a list of claims based on total incurred greater than 10,000 or all back injuries.
  • Reports - The link will allow the user to print standard loss runs and other pre-built reports as well as run some demographics reports. Demographic reports allow the user to view claims by various groupings, such as body part, day of the week, nature of injury, etc. You can also drill down and view the claims that make up each grouping.
  • Download Your Claims as a Text File - If you prefer to work with your data in Excel or some other analytical tool you can download the basic claim detail into a tab delimited text file by selecting this link. You will be prompted to save the file and provide a name. It is recommended that you change the name to something more meaningful. Once the file is saved you can import the data into most any spreadsheet or database program.

How do I submit a Payroll Report?

Once you have logged on to Montlake's On-Line Services you will see the On-Line Services Menu. Select the "Payroll Reporting" option. If you have more than one policy with Montlake you will be prompted to select the policy number for which you are reporting payroll. Select "Submit" to continue.

Once you have selected the policy a payroll reporting form will open. This form will list all the class codes for each state and kind combination on your policy. You will need to input the payroll report period first, then using your TAB key you can navigate through all the class codes. You need only enter the payroll amount for each class code, the premium will calculate when you click the Calculate button. Once you have entered all the payroll values you will need to complete the bottom portion of the form. This portion includes the date you submitted the report, your e-mail address and any comments you would like to make. It is important that your e-mail address is entered correctly, as the system will send you a confirmation to this address.

Once the data is entered you can click on the "Calculate" button. This will calculate the total earned premium for the report and display it accordingly. Once you are satisfied with the data entry, you can print a copy of the report for you records, by clicking on the "Print" button. When you are ready, click on the "Send Report" button, and a copy of this data will be sent to Montlake, along with an e-mail letting the Underwriter know that the report has been submitted. You will receive a confirmation e-mail as well.

You can submit as many reports as you like. If you submit more than one for the same report period, you may want to enter a comment to help distinguish between the multiple reports. If the report is a correction to a previous report it would be helpful to indicate that. If you have multiple locations reporting, it would be helpful if they indicate which location the report is for.
